In Joel Spolsky's article "Choices = Headaches", Joel went through the design of the Windws Shutdown sequence and whittled it down to, basically, "Let me go away from the computer". He wondered what could possibly have created the confusing mess that seems to have become part of the Windows shutdown mess in Vista. Well, he got his answer: moblog: The Windows Shutdown crapfest. I can only shake my head in disgusted agreement at how design meetings can turn into complete crapfests. It's enough to make grown programmers cry.
